PHQ9 - Depression Questionnaire
The PHQ9 screening tool allows Dr Taiwo Akindipe to see at what level you feel your depression is. It is a repeating questionnaire that can be completed before every appointment.
It is important to note that the PHQ9 is not a diagnostic tool. The results cannot be seen as conclusive without interpretation from a clinician, in this case, Dr Taiwo Akindipe. The PHQ9 functions as a screening and monitoring tool, which is the one-sided answers from the patient. Results can be impacted by the state of mind, intentions and limited frame of reference of the person answering the questionnaire.
Complete this questionnaire as honestly as possible, in a place where you won’t be disturbed.
