Linked below are all of the medical aid forms available to us for 2024.
PMB stands for Prescribed Minimum Benefits. These are conditions that the Council of Medical Schemes have in place to ensure that all medical aid members have access to essential healthcare services. Basically, these benefits cover some of the consultation fees and may cover some of the chronic medications. Each treating clinician needs to apply for PMBs on an annual basis for each patient. Therefore, you will need to complete these forms each year at the beginning of the year.
By completing and sending us these forms, you are aware and consent to your diagnosis code being shared with your medical aid to be able to apply for this benefit.
Consequently, these forms remain your responsibility to complete and follow up with your medical aid.
Your Steps to Apply:
- Select and download the medical aid form that is relevant to you.
- Complete your personal details and sign only where the patient’s signature is required.
- After completing the forms, email the form in pdf format back to clincic@mtobrain.com.
After receiving the pdf forms, we will complete the clinical details and send them to the relevant medical aid.
